Crossroads Group Home
Crossroads Group Home responds to the needs of the community by providing a safe and nurturing place to live for foster youth in crisis. Crossroads helps to stabilize and prepare residents for reunification or a longer-term placement. Crossroads does this by assessing resident needs, helping them increase their ability to function and solve problems while coordinating with other appropriate agencies.
Crossroads Group Home is a co-ed, 6-bed, level-10 group home for youth 12-17 years old. The program creates a stable, short-term living environment for adolescents experiencing difficulty in placement or with homelessness. All referrals come from Children and Family Services, including youth who may:
- Have a history of mental health issues and/or failed foster placements.
- Are removed from their home and require immediate emergency placement.
- Are homeless and need a stable living situation.
- Are placed out of Santa Cruz County and are re-entering the County.
The three main components of the Crossroads Program are Residential Services, Intensive Mental Health Services and Coordination with Child Welfare Services and Children’s Mental Health staff who work with our target population. Residents are fully assessed in all areas of functioning to determine strengths and weaknesses and to develop an appropriate treatment plan.
Who Qualifies? Male and female youth aged 12 – 17. All referrals are made by County Child Welfare Services
