CSS Coordination Team
The Mental Health Services Coordination team provides case management and crisis intervention services to individuals with a dual diagnosis of co-occuring mental health and substance abuse disorders. The CSS team is a Full Service Partnership Team, utilizing a modified Assertive Community Treatment (ACT) model with caseloads averaging 1:20, and 24 hour, 7-day availability to individuals on their caseload.
CSS Coordinators work with individuals to assist them in maintaining psychiatric stability and sobriety, and in developing support networks and connecting to the community. Services focus on wellness and are strengths-based. Coordinators assist clients in meeting personal goals related to education, employment, and housing, as well as pursuing talents and areas of interest such as music, art, yoga, spirituality, etc.
